You can refer yourself simply by contacting me. You can phone me and leave a message (with your name, phone number and the best days and times for me to reach you). I will then call you back. When we speak, I will provide you with information about my services and answer any questions that you may have.

I will probably also send you some information and forms to complete. If you book an appointment to see me, you would send back the completed forms prior to the first appointment. My address and phone number are shown here :

For Professionals:

You can refer a client to me by phone or by fax. See above for the numbers. The minimal information I need is the client's name, phone number, date of birth and reason for referral. This assumes that the client has consented for you to release this information to me. I will then contact the client.

Alternatively, you can give the client my contact information, and he or she can contact me directly, as outlined above.
